Ready to rent Lucky Image Photo Booth? Please fill out the the intake questionnaire below so I can can ensure the photo booth is successful at your event. Please note filling out the below form DOES NOT guarantee the photo booth will be available for your event. A confirmation email will be sent out along with a contract that will be issued on behalf of both parties for bookings agreement.

Payment Policy:

An $100 non-fundable deposit is due in order to secure the date for your event and will go towards final payment. Information on where to send deposit will be sent with the confirmation email once the intake form has been filled out. The date for your event cannot be held until the deposit has been sent. Final payment is due TWO DAYS before the event.
